As a caregiver, staying connected with the care team is essential. You may have questions about treatment plans, need updates on a patient's progress or need specific medical information.

This page shares how you can contact health care providers at UHN. There are also services that offer help in your local community and connect you to providers for health advice when you need it most.

UHN Services

The UHN Switchboard directs all phone calls to the appropriate area, whether it's a clinic, unit or a specific provider.

Call the Switchboard at your UHN site to be transferred to the right extension:

  • Princess Margaret Hospital: 416 946 2000
  • Toronto General Hospital: 416 340 4800
  • Toronto Western Hospital: 416 603 2581
  • Toronto Rehabilitation Institute: 416 597 3422

Some doctors have a page that includes their phone number and sometimes their email.

Clinic pages have information about the clinic, its hours, location and phone number.

When you are home, medical issues can arise. It can be hard to know when to come back to UHN or seek care in your community.

Toronto's Virtual Emergency Department offers virtual visits for urgent, but not life-threatening, conditions. Book a same-day appointment in the morning to meet with a provider using your phone, tablet or computer.

During a virtual emergency department visit, a nurse practitioner will talk to you about your concerns and let you know what to do next.

If all appointments are full or you feel you can't wait, go to your nearest Emergency Department or Urgent Care Centre in person.

The Princess Margaret Cancer Centre Patient Information Line provides practical information and support in real-time for patients and their caregivers.

This service is available Monday to Friday, between 8:00 am and 4:30 pm.

Provincial Services

Health 811 is a free, secure, confidential service available 24/7 to find health services or information or get health advice from a registered nurse.

The person you speak to will ask questions to understand your needs and give appropriate advice on your situation. They can help you decide whether you can handle a problem yourself, visit your doctor, go to a clinic, or go to an emergency room.

This service is confidential. You may be asked to provide a health card number, but it is not required.

This service is provided in both English and French, with translation support offered in other languages.

Ontario Health atHome coordinates local home and community care and helps people with Ontario's long-term care home placement process.

They assess patient care needs and deliver in-home and community-based services to support your health and well-being.

Who is eligible?

Ontario residents with a valid Ontario Health Insurance Plan (OHIP) card can be assessed by a care coordinator.

Based on your assessment, they may provide you with the following services:

  • Nursing
  • Personal support (bathing, dressing, etc.)
  • Physiotherapy
  • Occupational therapy
  • Speech-language therapy
  • Medical supplies and equipment (some fees may apply)
  • Long-term care placement
  • Social work
  • Nutrition counselling
  • and more
