Access Services

Learn about the different UHN services you might need during your caregiving journey


UHN offers a range of support and services designed to provide practical assistance, information and support.

Ask about these services during your hospital visit to access the resources that meet your needs before leaving UHN.


Your health care team may include:

  • doctors
  • specialists
  • nurses
  • pharmacists
  • physiotherapists
  • dietitians
  • occupational therapists
  • social workers
  • psychologists
  • other professionals

The care team works together to make care plans and give expert advice specific to a patient's needs.

If you would like to speak with one of these professionals and they have not already been assigned to you, ask your care team if a referral is possible.

Qualified medical interpreters and translators help you communicate with the care team in the language of your choice. This is to help make sure you understand treatment plans and medical instructions.

UHN interpreters work in over 180 languages, including American Sign Language, French, Cantonese, Italian, Mandarin, Portuguese, Spanish, Vietnamese and more.

A patient can add their preferred language to their health record so their providers know in advance.

Interpreters are available in person and over the phone. For short appointments, follow-up visits and diagnostic tests, an over-the-phone interpreter will be called. For long appointments, an in-person interpreter will be called.

myUHN Patient Portal is a free and secure website or app for UHN patients.

On myUHN, you can see appointments, update personal information, access health records and lab results, send messages directly to select clinics and complete questionnaires before appointments.

UHN Health Record Services provides patients with a copy of their health records and medical images from their time at UHN.

Patients must complete and sign a disclosure form and pay an administrative fee. Patients can share their records with a relative, friend, family doctor, nurse practitioner or another institution by signing an authorization form.

Patients with a Substitute Decision Maker (SDM) and/or Power of Attorney (POA) can share the official documents with Health Record Services to have them added to their UHN file.

Pharmacies are available to help you manage your medications, to provide advice on prescriptions and to answer questions related to medications.

Outpatient pharmacies are available at many of our sites.

Inpatient pharmacy services are available throughout a stay at UHN. If you wish to speak to an inpatient pharmacist during your stay, ask a member of the care team.

The Patient Relations team can support you with any concerns or questions about your care experience. They can help resolve issues and improve communication with the hospital.

Whether you have questions, a complaint, a suggestion or a compliment, contact Patient Relations to speak to an Advisor.

Before contacting Patient Relations, please speak to your care team. It's your right to ask questions and express concerns about your experience at UHN. Your care team may be able to act on your concerns and answer any health care-related questions right away.

The UHN Privacy team is committed to protecting the privacy of patients and their personal health information by following Ontario laws. This means that UHN treats health information with respect and sensitivity.

Access to personal health information is only available to those who need to know to care for a patient.

If you have questions or concerns about how health information is used, and who UHN shares your health information with, the privacy office can help.

A bioethicist is someone who supports staff, patients and caregivers in dealing with ethical issues in a patient's care.

Bioethicists give patients and their caregivers the information and support they need to make informed treatment decisions. They help identify common interests and resolve conflicts when parties disagree about patient care.

For example, bioethicists are often contacted to help Substitute Decision Makers when a patient can't express their wishes.

If you have questions about consent, confidentiality, or ethical concerns related to care, a bioethicist can provide guidance and support.

Call a bioethicist from any UHN site from 9 am to 5 pm on weekdays.
