Referrals

Referrals are only accepted from hospitals, physicians, or nurse practitioners.

To submit a referral, please complete the referral form and fax it to 416 243 3696. Please attach recent progress notes and public health lab results.

If you are using the Resource Matching and Referral process (RM&R), please also send the TB Outpatient Referral form.

After we receive your referral, our team will review, and the Service Coordinator will contact the referral source.

What We ​Do

  • Assessment of new referrals suspected of having TB with recommendations for community and/or hospital management
  • Ongoing treatment and follow-up of clients with pulmonary and extrapulmonary TB
  • Assessment and treatment of clients with non-tuberculosis mycobacterial infection​
  • Investigation of clients who test positive for TB
  • Treatment and follow-up of latent tuberculosis infection.​

Eligibility Criteria

Patients of West Park's Tuberculosis Service are 16 years of age and older with a suspected or confirmed diagnosis of tuberculosis or other mycobacterial infection. In-patient treatment may be required if the patient has:​

  • Drug resistant or multi-drug resistant strains of TB
  • A reaction to medication
  • Associated conditions that make out-patient treatment not possible
  • The inability to self-isolate at home or living with immunocompromised people.
