Referrals
Referrals are made via the RM&R Centralized LTV Waitlist (LTVHub) for ICU's within the Greater Toronto Area.
For referrals outside this area, please download and fax or email admitting@westpark.org the appropriate referral form to West Park's Admitting Department.
What We Do
Because of illness or injury, some individuals are unable to breathe on their own. West Park's Long Term Ventilation (LTV) service is an inpatient unit that provides care to people who need ongoing mechanical assistance to help them breathe for all or part of the day.
We provide care for these individuals, with a focus on improving their health, well-being and independence. In addition to meeting their physical needs, West Park staff members help patients develop the skills to plan and direct their own care and, in some cases, to explore independent living.
The LTV service offers both long-term residential care and slow stream rehabilitation (twelve weeks to two years) for patients able to reintegrate into the community.
Eligibility Criteria
Patients of West Park's LTV Service are adults (18 years of age and older) who:
- Require invasive ventilatory support due to chronic ventilatory failure
- Are non-smokers
- Are medically stable and no longer require continuous cardio-respiratory monitoring and/or central venous access.
Costs
The cost of medical and professional care is covered for patients with a valid Ontario Health Card issued by the Ministry of Health.
The Ministry of Health requires an additional daily complex continuing care co-payment, which is subject to change (a 30-day notice of increases will be provided).
