Referrals
Referrals are only accepted from hospitals or physicians.
To submit a referral, you may use the Resource Matching and Referral process (RM&R), or download and complete the Inpatient Rehab/CCC Referral Form (a standardized GTA Rehab Network form).
Once you have completed the form, fax it to 416 243 8397.
After we receive your referral, our Discharge and Flow Coordinator will contact the referral source.
What We Do
West Park's Geriatric Low Tolerance Long Duration (LTLD) service helps older adults improve their functional ability and quality of life after an injury or stroke.
The service provides longer-term and less intensive rehab and therapeutic services to geriatric patients. It is designed to help older patients (age 55 and older) who have sustained significant illness or injury and who require more time and less intensive therapy to improve their functional ability, level of independence, and quality of life
Patients are admitted for a 90 day period or less. For patients with hip fractures, the length of stay is 60 days or less if the patient achieves their goals.
A transdisciplinary team of specialized functional enhancement professionals provides a comprehensive assessment to establish a treatment plan that incorporates both therapeutic and patient goals. This plan may include:
- Active therapy to improve health, well-being, and quality of life
- Individual and/or group therapy sessions three to five times each week, based on individual needs
- Working with patients and families throughout the program to identify community resources and services that may be of assistance to patients upon their return to community living.
Eligibility Criteria
Candidates for the Geriatric Low Tolerance Long Duration service are adults who:
- Are 55 years of age and older
- Are medically stable and have identified therapy goals prior to admission in at least two areas (physiotherapy, occupational therapy, speech language pathology, etc.)
- Have sufficient cognitive capacity to participate in goal-setting and are able to learn and retain therapy instructions/information (They have the ability to follow 2-step instructions)
- Are able to tolerate and benefit from regular therapy interventions (20-30 minutes per day)
- Are motivated and willing to take part in programs offered
- Have an identified discharge destination (e.g., their own home, a retirement residence).
Costs
The cost of medical and professional care is covered for patients with a valid Ontario Health Card issued by the Ministry of Health.
