Referrals

Referrals are only accepted from hospitals or physicians.

To submit a referral, you may use the Resource Matching and Referral process (RM&R), or download and complete the Inpatient Rehab/CCC Referral Form (a standardized GTA Rehab Network form).

Once you have completed the form, fax it to 416 243 8397.

After we receive your referral, our Discharge and Flow Coordinator will contact the referral source.

What We Do

West Park 's General Complex Continuing Care Service is a specialized 24-hour program for people with complex or progressive illnesses or disabilities, such as Multiple Sclerosis (MS) Lou Gehrig's disease (ALS), paraplegia or mild dementia.

We do not accept patients who display inappropriate behaviours such as verbal or physical abuse.

​The ​​Service Provides:
  • Care by professionals experienced in treating the complex medical, physical, social and emotional problems associated with long-term illness and disabilities
  • In conjunction with a healthcare team, a personalized program to meet specific care and treatment needs
  • Education about the patient's condition, including opportunities to interact with others dealing with similar issues
  • Encouragement for family and friends to be a valuable part of the patient's support network and partner in the patient's activities
  • Special sensory stimulation activities that help patients connect with their environment and live more richly within their limitations.

Eligibility Criteria

Candidates for the Complex Continuing Care​ are adults who:

  • Have a chronic or progressive illness whose potential for improvement is limited
  • Are medically stable
  • Are unable to live independently in the community
  • Have care needs that exceed those available through community services or in a nursing home because of medical complexity (e.g. tracheostomy or feeding tubes)
  • When a patient's care needs can be effectively managed in a community setting or a long-term care facility, we organize an interdisciplinary team meeting with the patient and their family to discuss and plan for discharge.
  • ​Please note that our CCC unit serves as a temporary care solution and is not intended to be a permanent placement.

Costs

The cost of medical and professional care is covered for patients with a valid Ontario Health Card issued by the Ministry of Health.

The Ministry of Health requires an additional daily complex continuing care co-payment, which is subject to change (a 30-day notice of increases will be provided).
