Referrals
Referrals are only accepted from hospitals or physicians.
To submit a referral, you may use the Resource Matching and Referral process (RM&R), or download and complete the Inpatient Rehab/CCC Referral Form (a standardized GTA Rehab Network form).
Once you have completed the form, fax it to 416 243 8397.
After we receive your referral, our Discharge and Flow Coordinator will contact the referral source.
What We Do
West Park inpatient Stroke & Neurological Rehabilitation unit is designed to help patients recover from a stroke or neurological condition so they may reach a level of independence to return to living in the community.
The service comprises of an interdisciplinary health care team including Dietitian, Neuro-psychologist, Service Coordinators, Nurses, Occupational Therapists, Pharmacists, Physicians, Physiotherapists, Recreation Therapists, Rehabilitation Assistants, Speech-Language Pathologists, Communicative Disorder Assistants, and Social Workers.
The Stroke & Neurological Rehabilitation service encourages family involvement in goal setting, treatment sessions, recreation activities, and discharge planning. Members of the care team will meet with patients, family, and friends to provide information and education on:
- Causes and effects of stroke or acquired brain injury
- Recovery and the healing journey
- Nutrition
- Medication
- Stress management
- Intimacy and Sexuality
- Mobility and devices to help with day-to-day activity.
A graduated discharge process begins with day passes and progresses to overnight stays and weekend visits as appropriate.
Patients may be referred for Outpatient Rehabilitation, which offers services to further improve progress and independence while living in the community.
Eligibility Criteria
Patients of West Park's Stroke & Neurological Rehab Service:
- Are adults (18 years of age and older)
- Have recently experienced a stroke or other neurological condition and would benefit from short-stay intensive rehabilitation
- Are medically stable.
Costs
The cost of medical and professional care is covered for patients with a valid Ontario Health Card issued by the Ministry of Health.
