Referrals
Referrals are only accepted from hospitals or physicians. To submit a referral,
download and complete this form.
All referrals must be sent to the Acquired Brain Injury Network (ABI Network) by fax at 416 597 7021. The network oversees referrals for all ABI rehabilitation facilities within the Greater Toronto Area and sends referrals to facilities based on the patient's choice.
Once we receive your referral from the ABI Network, a member of our team will contact the referral source.
What We Do
The inpatient Acquired Brain Injury Behaviour Service (ABIBS) is for individuals with an acquired brain injury who require rehabilitation in order to live in the community. The individual also has behaviours that put themselves or others at risk.
Using principles of Applied Behaviour Analysis to provide a blend of skill training and behavioural approaches, we work with patients to develop and implement individualized behavioural rehabilitation plans. Treatment is designed to transition clients to pre-identified community settings secured by the referring agency.
The service is located in wheelchair-accessible rooms featuring common kitchen and lounge areas with features to ensure safety for patients and staff.
Our team includes Behaviour Support Workers, Nurses, a Behaviour Analyst, Social Worker, consultant Psychologist, consultant Physician, consultant Speech Language Pathologist, Physiotherapist and Occupational Therapist.
Eligibility Criteria
Patients of West Park's ABIBS are adults (18 to 65 years of age) with an acquired brain injury as the result of a trauma or a disease process that has resulted in behaviours that put the person or others at risk. West Park may consider those over 65 years of age and have no dementia present.
Patients should also:
- Be medically stable
- Be under the care of a physician
- Not require immediate hospitalization for medical or psychiatric reasons
- Not be facing criminal charges.
Costs
The cost of medical and professional care is covered for patients with a valid Ontario Health Card issued by the
Ministry of Health
Community outreach behavioural services are also available to those with an ABI (Acquired Brain Injury) who currently reside in the community.
