Please connect with your Health Care Team while you are a patient in the Emergency Department.
I have insurance paperwork that needs to be completed by the provider I saw when I was a patient in the Emergency Department.

I have a compliment/concern from when I was a patient in the Emergency Department on a previous occasion.
