About UHN Reactivation Care Centre

UHN's Reactivation Care Centre (RCC) provides a supportive space, for specialized care, focused on reactivation, for patients who no longer need acute care and are focused on the goal of safely returning to the community – whether that be home, long-term-care, or an alternative home setting.

As part of the Ministry of Health and Ontario Health's commitment to alleviating hospital pressures across the Greater Toronto Area, the RCC, which UHN operates in partnership with Sinai Health and Trillium Health Partners, connects patients with the right care, in the right place, at the right time.

The UHN RCC will open up to 94 beds in summer​ 2024, with a plan to gradually increase occupancy to 188 beds.

Enhancing the Patient Experience

UHN's RCC is nestled in a residential neighbourhood, where quiet surroundings and green space extend our calm and healing environment.

While therapeutic programming keeps patients focused on achieving their individual reactivation goals – whether it's building strength and endurance, managing medication, or maintaining mobility, as they plan for a safe transition – our recreational schedule builds community, encourages socialization and connects patients to activ​ities that are meaningful in their everyday lives.

Together, they are committed to putting the needs of patients first, and offering an environment where patients feel safe, motivated, and prepared, as they plan for the next step in their care journey.

Who is eligible for this specialized service?

Each hospital has developed criteria to help determine which patients would benefit most.

This criteria will be clearly communicated to patients by their care teams at each hospital site, ahead of their planned transition from acute care to the UHN RCC. Teams will take the time to answer any questions patient may have and help ensure the transition is as seamless as possible.

At the UHN RCC, each hospital will occupy their own unit, where staff from their own hospital will offer direct patient care.
