Referrals

Referrals for this clinic are processed by UHN's Centre for Mental Health Central Intake Referral system. Submit the completed form via fax (416 603 5215). The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you. Please note that only physicians and nurse practitioners may send a referral.

After you have referred your patient, our admin team will contact the patient directly with the appointment information. Information about the appointment will also be sent to your office as per the CPSO guidelines.

Who is eligible?

Referrals are accepted for patients with specific neuropsychiatric issues including sleep disorders, seizure disorders (like epilepsy), movement disorders (like Parkinson's, Dystonia, Functional Movement Disorders), rheumatological conditions (e.g. lupus, rheumatoid, psoriasis) or acquired brain injury (motor vehicle accidents, etc.).

The service does not currently provide support to patients experiencing somatic symptom disorder, pain disorder, stroke, migraine/headaches, or Multiple Sclerosis (MS). Additionally, patients whose primary reason for referral is fibromyalgia are also outside the scope of our support at this time.

Unfortunately, due to limited resources, we cannot accept referrals involving active litigation (including but not limited to MVAs and WSIB), consultations for the purpose of submitting information for immigration-related issues, disability applications or forensic assessments. Additionally, referrals primarily concerning ADHD, autism spectrum disorder (ASD) or developmental delay will not be accepted. Contact our central referral intake office if you are unsure as to whether or not a referral is appropriate.

What We Do

We provide psychiatric care to patients experiencing mental health illness within the context of a neurological disorder. Treatment approaches will vary by disorder and can range in duration from one-time consultation to short-term follow-up. Our care is provided primarily in outpatient settings but also in inpatient settings where we collaborate with Consultation-Liaison Psychiatry to provide mental health support to patients undergoing complex medical or surgical interventions.
