In the absence of your correct contact information, UHN cannot guarantee timely and accurate delivery of your patients' reports to your office.

UHN's Provider List (Physicians and Nurse Practitioners) will be updated in real-time, through internal processes, and annually, through the College of Physicians and Surgeons of Ontario (CPSO).
