To submit a referral from a hospital, you may use the Resource Matching and Referral process (RM&R), or download and complete the referral form. For doctor’s offices referring patients from home, please fax all documents to 416 243 3696. For hospitals referring patients in acute care, please fax all documents to 416 243 3900.

What We Do

West Park's Respiratory Rehabilitation inpatient program is a 5-6 week, high intensity, group- based service designed to help patients with chronic lung impairment improve their ability to live independently in the community.

This service uses an interdisciplinary team-based approach to provide education, planning and coping skills, and a supervised exercise program that includes breathing control techniques.

The Respiratory Rehabilitation service provides:

  • Assistance to achieve as much independence as possible, based on personal goals established prior to and during the hospital stay
  • Patient education about lung disease and medications, as well as how to plan and cope with day-to-day activities and independent living in the community
  • A supervised exercise program that focuses on cardiovascular fitness, upper and lower limb strength, endurance training, energy conservation, relaxation training, breathing control and coping techniques
  • Weekend leaves so patients can practice acquired skills at home​.​​

Eligibility Criteria

The inpatient Respiratory Rehabilitation program is available to people who are currently managing at home, and to those who are currently in hospital who have been referred by a Physician or Nurse Practitioner.

Patients of the Respiratory Rehabilitation service are adults (18 years of age and older) who:
  • Have some sort of chronic lung impairment such as Chronic Obstructive Pulmonary Disease, Interstitial Lung Disease, Pulmonary Fibrosis, Chronic Respiratory Failure, COVID lung damage, etc.
  • Are medically stable with no cardiac concerns
  • Are mentally stable with no active addiction concerns)​​​​​
  • Patients must be independent with bed mobility, transfers, toileting, and walking short distances, using a rollator or walker, as necessary
  • Are cognitively intact and able to retain new information
  • Have the potential to improve their health and/or their independence
  • Are non-smokers (or at least willing to quit smoking while at West Park)
  • Have a discharge destination/are able to return to living in the community upon completion of the 5 to 6-week program​

Costs
