To submit a referral from a hospital, you may use the Resource Matching and Referral process (RM&R), or download and complete the referral form.
For doctor’s offices referring patients from home, please fax all documents to 416 243 3696. For hospitals referring patients in acute care, please fax all documents to 416 243 3900.
What We Do
Members of the interdisciplinary West Park Healthcare Centre team will assess and teach patients how to use non-invasive ventilation - i.e. BiPAP, mouth piece ventilation, etc. The team may also suggest modifications for community living, mobility, and communication devices to help patients live as comfortably, safely, and independently as possible.
This service provides:
- Assessment and training on the use of non-invasive ventilation equipment to treat breathing issues related to conditions such as sleep apnea, COPD, and neuromuscular disease
- Assistance with arrangements for community support services
- Access to Ministry of Health funding for ventilatory support equipment
- Patients usually return for yearly follow-up reassessments; short-stay re-admission may be offered if necessary.
Eligibility Criteria
Candidates for West Park Healthcare Centre’s Home Ventilator Training Service are:
- 18 years of age and older and require non-invasive ventilation nocturnally or for part of the day
- Willing to participate in the program
- Medically stable
- Cognitively stable (or have a documented substitute decision-maker)
- Able to direct their own care
- Able to return to the community with established discharge support.
Costs
The cost of medical and professional care is covered for patients with a valid Ontario Health Card issued by the Ministry of Health.
The Ministry of Health requires an additional daily complex continuing care co-payment, which is subject to change (a 30-day notice of increases will be provided).
