Seniors Mental Health Service​​

West Park’s Seniors Mental Health Service helps adults 60 years of age and older with concerns about depressed mood, memory loss, anxiety, stress, and/or difficulty managing everyday activities. ​

We provide in-home assessment to older adults living in Etobicoke, York, and North York (west of Dufferin Street). If you live outside of these areas, we can suggest a similar service in your neighbourhood.​

Our team of professionals provides practical advice and information about coping with mental health symptoms in order to maximize client quality of life. We recommend community programs and other mental health services and we help clients to navigate the health care system.

Assessment is completed by a mental health professional such as a social worker, nurse, or occupational therapist. Family members and care partners are encouraged to be active participants in the process.

Our clinical staff consults with the team psychiatrist to review client challenges and to develop an individualized care plan. Care planning can also be coordinated with the family doctor and/or referral source/agency.

Seniors' Mental Health Services Include:

  • In-home mental health assessment, individualized recommendations, and care planning
  • Linkage to home and community mental health services and programs
  • Health education and advocacy to promote independent, active, and healthy lifestyles
  • Indirect consultation with psychiatry.

E​ligibility Criteria

Individuals 60 years of age or older who:

  • Are experiencing a mental health issue (or symptoms from a previously diagnosed mental health issue);
  • Display significant changes in mood and behaviour;
  • Have difficulty managing safely due to changes in their mental function;
  • Are medically stable;
  • Reside in the community (including a retirement residence).

This service is also helpful for care partners experiencing stress who would like information about local community services and/or long-term care facilities.

​Costs

There is no cost for this service. We do not offer crisis services or urgent medical care.
