Referrals

Referrals are only accepted from hospitals or physicians.

To submit a referral, you may use the Resource Matching and Referral process (RM&R), or download and complete the Inpatient Rehab/CCC Referral Form (a standardized GTA Rehab Network form).

Once you have completed the form, fax it to 416 243 8397.

After we receive your referral, our Discharge and Flow Coordinator will contact the referral source.

What We Do

West Park's Geriatric Rehabilitation service is a transitional program that supports seniors by improving functional abilities and enhancing their health and wellness.

The inpatient program is geared towards seniors whose goal is to return home and who can transition there within 4-6 weeks. Seniors can expect:​​

  • To be seen by our interprofessional team and Geriatrician
  • To work collaboratively with the care team and their family to develop goals that will enable them to return home to independent living
  • To be provided with referrals to community supports and geriatric follow-up as needed upon completion of the program.

West Park’s Geriatric Rapid Rehab offers a bridge to help seniors successfully transition from the emergency department and back to their homes. This high-intensity program provides a short stay (up to 14 days) with a specialized Geriatric Medicine and Rehabilitation team focused on helping you recover and get back on your feet.

Eligibility​ Criteria

Patients of the Geriatric Rehabilitation service are adults (60 years of age and older) who:

  • Have an established discharge destination in the community
  • Have recent acute decline/deconditioning, but not bed-bound
  • Are medically stable (not requiring acute intervention/monitoring)
  • Have the ability to tolerate a minimum of 45 - 60 minutes of therapy 4-5 days per week
  • Have sufficient cognitive ability to learn and retain information and motivation to participate in therapy with identified rehab goals.

Costs

  • The cost of medical and professional care is covered for patients with a valid Ontario Health Card issued by the Ministry of Health
