What We Do

The Geriatric Day Hospital is an o​utpatient rehabilitation program, designed to assist older adults who have experienced a functional loss and have identifiable rehabilitation goals. The functional loss may include a change in memory, mood and/or dealing with multiple health conditions. We focus on helping older adults living in the community to be more active and to maximize their function, independence and safety.

Our dynamic team of health professionals provides time-limited, OHIP covered rehabilitation to individuals with multiple health conditions. Our health professional include occupational therapy, physiotherapy, rehabilitation assistant, social work and nursing.

Our Servic​es

  • Detailed assessments and personalized treatment programs
  • Individual or small group therapy sessions with clear rehabilitation goals
  • Sessions may include: cognitive, physical or psychosocial assessment, health education, connections to community services and programs, hands-on exercise, discharge planning and support.

Program Hours, ​Attendance and Transportation

  • Mondays to Wednesdays, and Fridays​
  • Regular attendance is typically 1-2 hour sessions up to 3 times per week
  • Consultation by a Geriatrician can be arranged as approrpriate​
  • Maximum of 10 weeks. Clients who meet their goals sooner may be discharged before 10 weeks​
  • West Park does not provide transportation services. We can provide information about community transportation services, if needed.

Eligibility Criteria

  • A​dults 65+ living in the community with physical/cognitive/psychosocial concerns who are motivated to participate in rehabilitation
  • Participants must require two or more services (we do not accept referrals for Physiotherapy only)
  • Participants must be medically stable, able to manage their own continence and able to tolerate 45-60 minutes of activity
  • Individuals are able to follow instructions in English or be accompanied by someone who can provide translation
  • Adults with access to reliable transportation.

Exclusion C​riteria

  • Individuals who need more than minimum assistance (patients requiring assist must be accompanied by a care partner)
  • Older adults with cognitive, physical, behavioural or medical difficulties preventing participation in program activities
  • Residents of Long-Term Care
  • Previous admission to the Day Hospital in the last 2 years or recent discharge from an inpatient or outpatient rehabilitation program with no significant change in status.
