Referrals

Please note that a family doctor, nephrologist or dialysis centre may send a referral. For the standard referral process, please complete the referral form. For an electronic copy of the referral form, please email kidneytransplant@uhn.ca.

In addition to the completed referral form, please include all required tests:

Serologies

  • Varicella Zoster lgG
  • CMV
  • EBV
  • HCV
  • HIV 1&2
  • HTLV 1&2
  • HBsAg
  • HBsAb
  • HBcAB
  • Syphilis

Imaging

  • Chest X-ray
  • Abdominal U/S

Cardiovascular Assessments

  • 2D Echocardiogram
  • ECG
  • Stress Echo or Myocardial Perfusion Scan

In addition, the following information may be helpful in the evaluation process:

  1. Complete Medical History
  2. Letter of referral
  3. Copy of medication record
  4. Glucose tolerance test (all non-diabetics)
  5. Copy of hepatitis immunization
  6. Copy of most recent routine blood work (incl. Liver function and lipid profile)
  7. TB skin test
  8. All medical consults

Once we receive a referral package, we will call the patient within two to three business days. Our goal is to complete the patient's evaluation for kidney transplant within 90 days of receiving a complete referral package.

What We Do

Pre-Transplant
Patients being assessed for a kidney transplant will be seen through our FASTRAK Clinic located in the Ambulatory Transplant Centre. FASTRAK brings together your tests and evaluations into one place. This reduces the number of visits you make to the hospital.

There are 3 steps for a Kidney Transplant Assessment:

Step 1: Your nephrologist (kidney doctor) will send us a referral form and package which includes test and diagnostic results.

Step 2: You may be scheduled for two visits to the FASTRAK Clinic. The first visit will include tests and consultations with a transplant coordinator, social worker and physician. The second visit will include consults with the transplant nephrologist, and any other team members  who are required to meet your needs.

Step 3: The kidney transplant team will review all test results  to determine if a transplant is right for you.

 
Post-Transplant
When you go home from the hospital after your surgery, we will schedule your follow-up care in our Ambulatory Transplant Clinic.
