Note: Post-COVID Condition Rehabilitation Program only accepts internal referrals.

Toronto Rehab's outpatient post COVID-19 condition rehabilitation program is currently only accepting internal referrals from within UHN due to resource limitations.

Admission criteria includes:

  • Referred by UHN physician/nurse practitioner
  • COVID diagnosis (positive or presumed) with ongoing symptoms affecting day to day function and/or quality of life

What We Do

Toronto Rehab's outpatient post-COVID-19 condition rehabilitation program provides individualized rehab support to those experiencing ongoing COVID-19 symptoms impacting their day-to-day functioning.

Our program offers support to individuals with COVID-19 who continue to experience difficulties in the following areas:

  • endurance
  • balance
  • thinking skills
  • shortness of breath
  • speech and swallowing
  • managing difficult emotions
  • managing fatigue

An initial interdisciplinary virtual assessment is conducted and the team sets goals with the patient and tailors the program to meet their unique needs. The team provides education and guidance on how to self-manage ongoing symptoms and maximize function.

Visits are offered virtually, although in-person and/or telephone follow-up sessions occur on an as-needed basis. The length of stay in the program ranges between 2-12 weeks, depending on the individual's goals and how much they are benefiting from the program.
