Referrals

All referrals must be made through the Acquired Brain Injury (ABI) Network which oversees referrals for all ABI rehabilitation facilities within the GTA. The network sends referrals to appropriate facilities based on client choice.

Download the referral form from the Toronto ABI Network.

The Brain Injury Service has a single-point entry system. All referrals are screened by program service coordinators. The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you.

Our office will call the patient with information about the patient's first appointment.

What We Do

Our service provides outpatient care at both the University Centre and Rumsey Centre.

A brain injury can happen because of a car accident or a fall. Or it may be caused by a medical problem such as a stroke, brain aneurysm, brain tumour or cardiac arrest. Whatever the cause, a brain injury can be devastating and may lead to trouble in cognitive ability, moving, balance, coordination, speech and language, or behavioural and emotional problems.

Our service provides rehabilitation to people who have acquired non-degenerative neurological disorders. We help those with a brain injury work towards returning to work and to living more independently.

We tailor rehab programs to an individual's needs and focus on bringing back functions that can be restored. When that isn't possible, we teach people how to do things differently.
