Referral Forms

For urgent referrals, please call the Pulmonary Function Lab:
Toronto General Hospital
Phone: 416 340 4086
Toronto Western Hospital
Phone: 416 603 5377
We will accommodate urgent referrals as soon as possible.

Please note that only physicians may send a referral. Please download and complete the Pulmonary Function Test Requisition for either Toronto General Hospital (TGH) or Toronto Western Hospital (TWH).

The pulmonary function lab administrative assistant/ward clerk will call the referring physician's office with information about the patient's appointment.

The referring physician's office is responsible for providing the patient with the appointment information, including time and location, and important documents. Occasionally we will call the patient directly to book an appointment.

Depending on the test that is ordered, test prep instruction may be needed. Please ensure that your patient is aware of these instructions as the test may need to be cancelled and rebooked if not followed.

What We Do

Pulmonary function tests are breathing tests that are ordered by a physician and used in the assessment and management of lung disease. While sitting in a chair, the patient performs various breathing procedures through a mouthpiece connected to pulmonary measuring equipment. The full breathing test usually takes about 45 minutes to complete, but additional time is required if other specialized tests are also ordered.
