Referrals

If your patient needs immediate, urgent care, please send them to the emergency department. For urgent VT referrals, please fill out and fax the Complex Ablation Referral Form. After 5:00 pm, please page the EP physician on-call through locating 416 340 3155.

Our office will call you with information about the patient's first appointment. Our office is responsible for providing the patient with the appointment information, including time and location, and booking any pre-consultation tests and imaging required.

What We Do

Complex ablation​ procedures are sometimes used in order to help control certain types of arrhythmias. This is both an assessment as well as follow-up clinic. We see patients with complex arrhythmias to determine whether their conditions are suitable for certain procedures.

If you're accepted, you'll be seen again in this clinic for follow-up about 2 months after your procedure. All pre-screening is done before the appointment, and when you have your consultation, we encourage you to have a complete discussion with our doctors and staff about what's involved.

The team including physicians, nurses, allied health professionals and administrative staff are part of the Electrophysiology Department in the Peter Munk Cardiac Centre, where world-class services for the diagnosis, assessment and treatment of heart rhythm disorders are provided.​
