Referrals

Referrals for this clinic are processed by UHN's Centre for Mental Health Central Intake Referral system. Submit the completed form via fax (416 603 5215). The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you. Please note that only Primary Care Providers (PCP) including physicians and nurse practitioners may send a referral.

After you have referred your patient, our admin team will contact the patient directly with the appointment information. Information about the appointment will also be sent to your office as per the CPSO guidelines.

Who is eligible?

Patients experiencing mood disorders (anxiety, depression, bipolar), OCD, PTSD and psychotic disorders (psychosis, schizophrenia, schizoaffective disorder) seeking mental health and addiction services in Portuguese. Additionally, those with depression, anxiety, insomnia or chronic pain may be referred to the Integrative Behavioural Group Therapy (IBGT) .

There is no catchment area for patients who are Portuguese-speaking.

The clinic is unable to provide care to patients already under the care of a psychiatrist or at a facility offering psychiatric services. Nor is it able to provide cognitive assessments, treatment for patients suffering head injuries or individuals seeking referral exclusively for anger management.

Unfortunately, due to limited resources, we cannot accept referrals involving active litigation (including but not limited to MVAs and WSIB), consultations for the purpose of submitting information for immigration-related issues, disability applications or forensic assessments.

Additionally, referrals primarily concerning ADHD, autism spectrum disorder (ASD) or developmental delay will not be accepted. Contact our office if you are unsure as to whether or not a referral is appropriate.

What We Do

Our outpatient program provides mental health and addiction services to people who speak Portuguese. As we are a teaching hospital, you could also be seen by a resident in psychiatry. In collaboration with Outpatient Mental Health psychiatrists, Mental Health Clinicians provide culturally competent and time-limited therapeutic interventions in Portuguese.

Portuguese Mental Health and Addiction Services are staffed clinically by a psychiatrist and mental health clinicians as well as our administrative assistant. All of our staff provide services in Portuguese.
