Referrals

There are two different methods of referring a patient:

  1. Central Intake Referral Service

    Download: Central Intake Referral Form

    Submit the completed form via fax (416 603 5215). The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you.

    After you have referred your patient, our admin team will contact the patient directly with the appointment information. Information about the appointment will also be sent to your office as per the CPSO guidelines.

  2. Self Referral

    Patients can call our administrative assistant at 416 603 5025 to arrange a phone interview.

Who is eligible?

Patients are referred primarily due to substance or alcohol use concerns. Individuals should be capable of abstaining from substance use for 12 hours before appointments and willing to engage in outpatient counselling and psychotherapy specifically tailored for addiction treatment.

We are unable to support patients who:

  • Are currently under the care of another program or service providing psychotherapy/counselling for addictions.
  • Are primarily seeking support from a psychiatrist.
  • Demonstrate instability in substance use or mental health that could impede effective engagement in outpatient psychotherapy.
  • Have experienced severe cognitive impairment.
  • Seek assistance solely for nicotine use or behavioural addictions without concurrent substance or alcohol use issues.

Additionally, patients seeking medical support for addiction without a desire for counselling or psychotherapy should consider referral to the RAAM Clinic.

Unfortunately, due to limited resources, we do not accept referrals involving active litigation (including but not limited to MVAs and WSIB), consultations for the purpose of submitting information for immigration-related issues, disability applications or forensic assessments. Contact our central referral intake office if you are unsure as to whether or not a referral is appropriate.

What We Do

We are an outpatient program providing services to people over the age of 18 who are interested in making changes to their alcohol and/or other substance use. We offer time limited individual psychotherapy/counselling.

The program is voluntary, confidential and covered by OHIP.

Additional UHN addiction services are available through the RAAM Clinic.
