Referrals

Referrals are only accepted from physicians and nurse practitioners.

For referrals to the Toronto Western Hospital clinic, please use our general referral form located under the clinic's information. Please fax the referral to 416 603 5039.

For referrals to the Toronto General Hospital clinic, please fax your referral to the Gastroenterologist you wish to refer to.

Please include all relevant clinic notes, procedure reports, test results, imaging, etc. All referrals will be triaged and booked based on urgency and availability. Our office will call the referring physician with information about the patient's first appointment. The referring physician's office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring.

For referrals for Endoscopic Retrograde Cholangiopancreatography (ERCP), please fax a referral to Dr. Paul James.

What We Do

The Driscoll Family Digestive Health Centre is the leading centre for digestive health disorders in Canada. We have clinics located at Toronto General Hospital and Toronto Western Hospital. We care for a diverse population with varying complex digestive conditions. We are committed to providing excellent patient-centered care, outstanding education and training, access to transformational technology, and innovative research to align with UHN's vision for A Healthier World.

We are a multi-disciplinary team comprised of experienced gastroenterologists, nurses, and registered dieticians. We collaborate to provide comprehensive care to patients with complex digestive health disorders including: gastrointestinal (GI) cancers, irritable bowel syndrome, celiac disease, gastroesophageal reflux, GI motility & functional disorders, parenteral nutritional support, inflammatory bowel diseases, and advanced endoscopy.

Our goal is to educate patients about their condition, empower them to become active participants in their care, and to learn how to manage their condition along with our expert multidisciplinary team

How to Prepare

If you are unwell, please do not come to the hospital and contact the doctor's office to rebook your appointment.

Always bring a valid Ontario health card for ALL your appointments. Please bring a list of all current medications (prescribed, over-the-counter, and natural/herbal products) or the medications in their original bottles. Speak with your pharmacist to help you make a list, if needed.

For first appointments, please arrive 15 minutes before your appointment time to complete a patient intake questionnaire.
