Referral Information for Patients

You need a referral from your doctor to become a patient at the Neurophysiology Lab.

When making a referral, your doctor will include:

  • A letter of referral
  • Clinical notes
  • Test, x-rays and other relevant results

How Long Does a Referral Take?

Usually, the doctor who referred you to the Neurophysiology Lab will contact you with your appointment time. Call the doctor's office if you do not receive information about your appointment within a few weeks.

Referral Information for Physicians

Standard Referral

To complete the standard referral process, submit an Ocean eReferral or fax a completed form to the Neurophysiology Lab at 416 603 7096.

Our office will fax you information about the patient's first appointment.

Your office is responsible for providing the patient with the appointment information, including preparation instructions, time and location, and important documents, test results and imaging that they must bring.

Checklist for a Complete Referral

In addition to the completed referral form, make sure to include:

  • A complete list of medications your patient is taking

The form will be reviewed to ensure that it is complete. If this information is not included with the referral form, your referral may be returned as incomplete.

What We Do

The neurophysiology lab provides several tests to assist in the diagnosis and treatment of conditions of the brain and nervous system. Some examples of the conditions that we see are epilepsy, myasthenia gravis, and multiple sclerosis. We have a highly skilled staff of technologists who perform tests independently and in conjunction with physicians depending on what type of test is requested.​

What to Expect

Your Procedure

Your appointment will be sent to your referring physician as well as instructions for the day of the test. We will call you 2 days before to remind you of your appointment. Please ensure that your doctor has sent us your current telephone number.


What to Bring

  • CD or DVD of your imaging studies
    Your referring doctor may give you a CD or DVD of imaging studies such as X-rays, CT Scans, or MRIs you may have had done. If so, it is very important to bring this CD or DVD with you.
  • List of all medications you are currently taking
    This includes prescription medications, over-the-counter medications, vitamin or mineral supplements and herbal remedies.
  • Medications that you need to take
    Appointments can take several hours. Bring with you any medications you normally take during the day.
  • Record of any changes
    Make notes about any changes in your condition that you've noticed since your last visit, and bring these notes to show your doctor.
  • A trusted friend or family member
    A friend or family member can give you emotional support and can help you make good choices. They can also help you gather information, take notes and ask questions.
  • List of doctors and hospitals
    Bring a list of doctors you have seen in the past who may have information about your blood work. Also, bring the names of the hospitals where you have been treated.
  • Questions to ask
    Bring a list of questions to your appointment to help you remember everything you want to ask.

When You Arrive

The first person you meet at the Neurophysiology Lab is the receptionist. To sign in with the receptionist you will need your health card (OHIP card). If you do not have an OHIP card, please bring another form of government-issued photo ID (such as a driver's license, passport, or other provincial health card).

How Long Will Your Appointment Be?

First appointments take longer than follow-up appointments. Your first appointment can take 2 hours or more. After your visit, you will likely go to the Blood Collection Lab for blood tests.

Follow-up appointments usually take 15-30 minutes.

We do everything that we can to stay on time. Unfortunately, your appointment may be delayed by unforeseen circumstances. We recommend that you come prepared for delays.

Before You Leave

If you are unsure about what your next steps are, don't be afraid to ask a member of your team. We are here to help you. The results of your test will be communicated to your referring doctor.

About Phone Calls

We understand that reaching us by phone can sometimes be difficult. Often our phone lines are busy or are turned over to the message centre so our staff can prepare for clinic visits or help other patients.

We make every effort to return your call within 24 hours. Our staff will try to reach you 2 times. If we are not able to reach you directly – and you do not have an answering machine for us to leave a message – you may need to call us again.​
