Referrals

Now accepting Ocean eReferrals

This clinic is now accepting referrals through Ocean eReferral to help everyone save time and increase efficiency.

For urgent referrals, please call UHN locating 416 340 3155 and ask to speak with the Neurologist on call.

Please note that only physicians may send a referral. Please send an Ocean eReferral or fax the completed referral form to either Toronto General (416 340 4189) or Toronto Western (416 603 7096).

Our office will call the referring physician with information about the patient's first appointment. The referring physician's office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring.

Please note that clinic appointments may be virtual visits. The clinic team will let you know what type of appointment is appropriate for your care. Learn more about virtual visits at UHN »

What We Do

Our clinic specializes in the diagnosis and management of all neuromuscular conditions.

We care for patients with routine neuromuscular assessments such as carpal tunnel syndrome or radiculopathies, as well as complex conditions, including neuromuscular junction disorders such as myasthenia gravis, diabetic neuropathy, inflammatory neuropathy (CIDP/GBS) and muscular dystrophy. The clinic provides outpatient and selected inpatient consultations and follow-up for patients with chronic neuromuscular conditions.

Tests

  • Non-invasive small fiber / autonomic nerve testing
  • Percutaneous muscle biopsy
  • Quantitative sensory threshold testing

How to Prepare

Your first appointment can take 2 or 3 hours. Follow-up appointments usually take from half an hour to 60 minutes.
