Guide for Physicians Referring to the Genitourinary Clinic

​​​​​​​​​​Urgent Referral

If your patient needs immediate, urgent* care, contact the appropriate physician directly to complete an urgent referral.

*Note: Only testicular cancer and suspicious testicular masses are considered urgent.

Standard Referral

To complete the standard referral process:

Download and complete the applicable Genitourinary referral form.

Include the following documents:
  • A letter of referral including all patient demographics
  • Clinical notes
  • Tests, x-rays, CT, MRI and any other relevant results
  • Any time constraints by the patient
  • Any additional appointments you may have arranged for this patient pertaining to this consult request

Once you have completed the form, fax it directly to the physician. The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you.

Our office will call you with information about the patient’s first appointment.

Your office is responsible for providing the patient with the appointment information. This information includes time and location of appointment, and important documents, test results and imaging that they must bring. It is extremely important to ensure that your patient brings CD copies of their recent MRI, CT or Ultrasound. These copies will be required at their first appointment.

In addition to the completed referral form, make sure to include:

  • A letter of referral including all patient demographics
  • Clinical notes
  • Tests, x-rays, CT, MRI and any other relevant results
  • Any time constraints by the patient
  • Any additional appointments you may have arranged for this patient pertaining to this consult request

If this information is not included with the referral form, your referral may be returned as incomplete.
