Pre-Transplant
Patients being assessed for a liver transplant will be seen in the Ambulatory Transplant Centre. The liver transplant clinic brings together your tests and evaluations into one place. This reduces the number of visits you make to the hospital.
There are 4 steps for a Liver Transplant Assessment:
Step 1: Your hepatologist (liver doctor) will send us a referral form and package which includes test and diagnostic results.
Step 2: You may be scheduled for
two to four visits to the Liver Transplant Clinic. The liver transplant team will review all test results and interviews to determine if you will move forward with the liver evaluation.
The first visit will include tests and consultations with a transplant hepatologist. Additional visits will include seeing a anesthetist, social worker or psychiatry, and physician. A nurse coordinator guides you through this detailed evaluation process. Other visits will be arranged as needed with other healthcare professionals who are required to meet your needs.
Step 3: The liver transplant team will review all test results and interviews to determine if a transplant is right for you.
Step 4: Once approved, you'll be listed for a liver transplant.
Post-Transplant
When you go home from the hospital after your surgery, we will schedule your follow-up care in our Ambulatory Transplant Clinic.

How to Prepare
Your first appointment can take 1 to 3 hours or more. After your visit, you will likely go to the Blood Collection Lab for blood tests, you may also need to have a chest x-ray or echocardiogram. Follow-up appointments usually take 1 hour.
