A post-transplant coordinator is a registered nurse or nurse practitioner who works together with your transplant nephrologist to check on how you are doing after the operation. The post-transplant coordinator arranges your clinic visits and any tests you need after transplant. They are available to answer questions related to your transplant health concerns.
