Referrals

Please note that only physicians or nurse practitioners may send a referral. Please fax the completed referral form. Once our team receives all the needed information, a program assistant from our outpatient services will contact the patient with an appointment time.

What We Do

The Geriatric Day Hospital Service is an outpatient rehabilitation program that helps older adults with multiple health problems to be more active in the community.

We conduct detailed health assessments, develop personalized treatment plans for you, offer health education and put you in touch with community services and programs. We also provide comprehensive discharge planning and support.

As one of our patients, you'll have therapy structured around clear rehab goals. Therapy may include both 1:1 and group-based sessions. Our program lasts up to 10 weeks.

We also offer Learning the Ropes for Living with MCI®. This program is appropriate for patients with a diagnosis of amnestic mild cognitive impairment (MCI), and involves group-based education focused on lifestyle factors that can impact cognitive health and memory strategy training. This program is held once per week for 6 weeks.

Geriatric Day Hospital Transportation

Toronto Rehab does not provide transportation services to and from the hospital. If you are unable to access the hospital on your own, we can connect you with community transportation services such as Wheel-Trans. You can find out more about getting to and from UHN on our maps and directions page.

Patients of the Geriatric Day Hospital Service may have the following conditions or issues:

  • Balance issues
  • Dialysis
  • Difficulty walking
  • Falls
  • Mild cognitive impairment
  • Parkinson's disease
  • Parkinsonism
  • Stroke
  • Diabetes
  • Aphasia
  • Problems with swallowing
  • Frailty
  • Deconditioning
  • Failure to thrive

  • Physiotherapy and occupational therapy assistants
  • Speech-language pathology
  • Social work

We also help link patients to special services such as hearing assessments, foot care, dental care and vision care. But please remember the Geriatric Day Hospital cannot replace the care provided by your family doctor.​
