Referrals

Toronto Rehab's Cancer Outpatient Rehabilitation program is currently accepting referrals for individuals aged 18 or older with musculoskeletal and select neurologic conditions related to an oncology diagnosis or treatment.

To access the program you must:

  • be medically stable and ready to participate in higher intensity rehab therapy appointments
  • have rehab goals and the potential for functional improvement
  • have reliable transportation to and from your appointments
  • be independent with toileting/self-care (or have a caregiver/attendant accompany you for assistance)

What We Do

Toronto Rehab's Cancer Outpatient Rehabilitation (COR) Program helps individuals optimize their function and quality of life post oncology diagnosis and/or oncology treatment.

COR offers individualized assessments and treatment plans, based around clear rehab goals made in collaboration with your healthcare team.

Our program can support individuals with cancer-related concerns such as:

  • changes to function in areas of mobility and daily activities
  • deconditioning
  • myofascial tightness
  • cording
  • peripheral neuropathy
  • fatigue
  • cancer-related brain fog
  • changes to emotions and coping

If you have breast cancer and your healthcare team feel you may benefit from a group program, you may be referred to RESTORE. All eligible participants will be considered for this program automatically.

  • Reducing Effects on the Shoulder Through Oncology Rehab and Education (RESTORE) is an education and exercise program that helps people with breast cancer who have problems in the shoulder area. RESTORE can also help people with problems in the arm, chest or neck on the same side of the body.
  • The program teaches participants to manage common side effects of cancer surgery, chemotherapy or radiation. It combines education and exercise to improve pain and movement.
  • Participants will be enrolled for a 6-week session in a small group that includes education sessions as well as the option of pool therapy in a warm water pool.
