Referrals

Internal Referrals

All referrals must come from UHN physicians. Providers will need to complete a consult order within Epic.

External Referrals

Referrals sent to Behavioural Supports Ontario (BSO) will be forwarded to our team and transcribed into our system. All referrals must come from the physicians, social workers, occupational therapists or other members of the interprofessional team at acute and post-acute care facilities within the Toronto Central Region, including: all UHN hospitals, CAMH, Providence Healthcare, Reactivation Care Centre – Church Street (and Sunnybrook and St. Joseph's units), Salvation Army – Toronto Grace Health Centre, Runnymede Healthcare Centre, Toronto Grace Health Centre, West Park Healthcare Centre and Women's College Hospital.

What We Do

We are a mobile outreach team serving hospitals within the Toronto Central Region. The team is partnered with Behaviour Supports Ontario (BSO), where their mandate is to work with older adults and their families supporting loved ones experiencing responsive behaviour in acute care hospitals, in the community as well as in long-term care facilities. UHN's BSS team fills service gaps throughout the continuum of care for older adults with responsive behaviour and facilitates effective transitions between hospital and community settings when necessary. Our behaviour support model is based on a non-pharmacological approach personalized to fit each patient's needs. We focus on behaviour management and stabilization, supporting successful transitions, systems navigation, education, capacity building and advocacy.​
